3. Fill out the form below and attach your completed Room Request Form.
4. Once the event has been scheduled (you will be contacted), download and complete the Event Setup Form (Microsoft Word format). Fill out the form below and attach your completed Event Setup Form.

If you are unable to open the forms, you may get printed copies from the Parish Office during regular office hours.
