You can request a registration packet in one of three ways:
- Visit the parish office and pick up a packet.
- Call the parish office at (480) 899-1990 and ask for a packet to be mailed to you.
- Fill out the following on-line form, and a packet will be mailed or emailed to you.
After you have completed the packet, it may be mailed or dropped off at the parish office or the collection baskets at masses.
